Appointment Payment, Changes & Cancellations

Running Late / No Show Policy

Payment is processed online at the time of your booking. If your payment was not processed at the time of your booking, payment will be processed on your nominated credit card on the day of your scheduled appointment.

​

You can change or cancel your appointment up to 24 hours before your appointment time with no associated cancellation fees. 

A 50% refund will be processed when your booking is cancelled with less than 24 hours notice. 

We value your time and offer flexible scheduling options.

If you are late for a session, we will still need to finish at the original appointed time as other clients have sessions booked subsequently. 

Please note that we do not offer any refund if you are not showing up to your appointment.

Confidentiality Assured

At Form Counselling, we understand the importance of confidentiality in the counselling process. Your privacy is our top priority, and we adhere to strict ethical guidelines to ensure that your personal information remains secure. All interactions, from appointments to note takings are considered confidential and kept securely.

I require a written permission from my clients, should they wish to share any information with a designated third party or another agency.

Limits to the Private & Confidential agreement:

​

  1. You give written permission to share confidential information. 

  2. You threat to harm yourself or another.

  3. There is a suspected abuse of a child, disabled or elderly adult. 

  4. Some information shared that suggests the commission of a crime.

  5. Under a court order, when your file is subpoena.

  6. There are time when I consult with my peers/supervisors to discuss Topics raised in sessions in order to remain effective in servicing my clients - No clients' identifiable information are shared.

Communication - Feedback

Once you have booked a day and time for your session, that time is confirmed.

If you need to cancel or reschedule, please refer to the section above: "Appointment Payment, Changes & Cancellations".

Please note that my voicemail and email are confidential and protected as best I am able, but be aware there is a greater risk to confidentiality.

Please limit your phone conversation needs to appointment scheduling and emergencies as I am usually in sessions with clients during business hours.

​

If you are in crisis, please call 000 or refer to the list of numbers on this page.

Please note that I will not enter into telephone or email counselling except by prior arrangement.

Clients feedback and complaint procedures are integral to our commitment to exceptional service. I value the opinions of my clients and encourage open communication during our sessions.

 

When clients have any concern or complaint about the service, they can easily reach out to me either directly during the session or via email at info@formcounselling.com.au.
